There is a $20.00 non-refundable registration fee per student.  Save $5.00 if you register by September 1st.  Please complete the registration form below (one form per student) and send registration fees to Colleen's School of Dance, c/o Colleen Ensby, 323 County Route 57A, Phoenix, NY 13135, noting the student names with the payment.  Please note both registration fees and a completed registration form is required to register.
